Prevention Navigator
CurrentOverseeing coordination of outreach events, venue based testing, and safe space for program serving any and every black individual ages 13-35. Conducting counseling, testing, navigation, linkage, adherence/retention support of clients in HIV/STI/Hepatitis C (HCV) services through linkage and referrals into other Prevention programming and agency wide services, with an emphasis of serving key populations. Work with linkage team and other agency departments and partners, including medical providers, to actively test, link, and navigate clients to services including medical care and essential support services. Ensure compliance with Center for Disease Control/Office of Public Health (OPH)/funder requirements and attainment of program and outreach deliverables. Have a working knowledge and ability to navigate electronic health record systems. Assist supervisor and managers with meeting program area requirements, including data management, analysis and reporting, staff and staffing needs, supplies and all quality assurance (QA) measures. Ensure all reporting and evaluation mechanisms are complied with.
