Provides training and supportive services to people with severe mental illnesses living in the supervised living program.Develops, implements, and completes all required documentation, like ISPs and Quarterly Reviews, for all clients in the program.Assists consumers with daily living, medication management, general community support, residential case management services, and skills development.

Responsible for the coordination of daily activities related to intake assessment and referral of individuals seeking mental health services, both in the ER and office setting.Provides telephone screenings and initial clinical assessments, communicates findings of assessments to physician.Secures admission orders from physician and/or makes referrals to appropriate level of care.Provides insurance pre-certification when indicated for Thomas Memorial Hospital-based treatment programs.Follows up with referral sources and collects statistical data for departmental outcome studies.
