Case Manager
Current• Responsible for managing the cases of 18 – 23 children and their foster families by supervising the intake of each placement and providing support throughout the placement ensuring that each child’s physical, emotional, and cultural needs are met as reunification with biological family is pursued.• Maintain regularly audited files and recording relevant information into database. Maintain compliance with state regulations by writing over 90 detailed reports and treatment plans annually.• Collaborate with other stakeholders including DCYF social workers, court appointed special advocates, service providers, educators, and attorneys to ensure best outcomes for children in care.• Coordinate, schedule, and facilitate 15 monthly home visits with foster families and various care professionals.• Lead group trainings for prospective foster families to prepare them for the licensing process and represent quality foster care and reunification work in the community.• Teach and counsel families on the impact of trauma and how to use trauma informed care strategies to best support the child in their care.• Provide 24/7 crisis management and emergency support for foster families.
