This means that millions of FORMS 1095-A must be generated and mailed out. Considering the amount of data that must be process, it is understandable that errors sometime occur. In 2015, about 20 percent of all tax filers with FFM coverage received a Form 1095-A with errors.Some of the type of errors that occurred was.1.) Unreconciled data between the FFM and Health plan issuers(Policy issuer)2.) Information changes not appropriately reported to the FFM3.) Partial premium amount payments4.)
