In addition the system has 2 inpatient psychiatric and 4 post-acute care facilities as well as an extensive clinical lab service, a state wide home health agency and the recently formed Hartford HealthCare Medical Group. As CMO/VP was responsible for value based plans for 1800 provider organization. Developed and lead care coordination team for Integrated Care Partners comprised of Care Coordinator RN’s, Health Coaches and MSW’s . Oversaw all quality programs for Integrated Care Partners
