SMO GYNAECOLOGY ONCOLOGYWellington Regional Hospital SENIOR CLINICAL LECTURERUniversity of Otago, WellingtonOct 2018 – January 2023I came to wellington in 2018 to enjoy a change of scene and environment; I was one of two Gynaecology oncology Consultants working in a tertiary centre. Previously I was one of a team of three Gynaecology Oncology Consultants, who together with three Oncologists, two specialist nurses and the MDT provided tertiary level cancer care for a population of 1.7 million from the area of Lancashire and South Cumbria.Having originally trained in open surgery to deliver the full range of Gynaecology Oncology, I was appointed in 1998 to initiate the set-up of a tertiary service in Preston. Over the next 20 years I extended my training to include laparoscopic surgery and supra-radical surgery for ovarian cancer. Very recently I began the simulation part of Robotic training in the UK but there has been no opportunity to continue in New Zealand.I am competent to deliver ovarian cancer surgery, including upper abdominal surgery, supra-colic omentectomy, peritoneal stripping of the diaphragm and para-colic gutters, and in combination with colorectal surgery colleagues large and small bowel resection.I can undertake advanced laparoscopic surgery for pelvic cancers and laparoscopic pelvic and para-aortic lymph node where indicated; I can perform radical vulval surgery, with some ability in plastic skin flaps (more extensive flaps done in collaboration with plastic surgery colleagues) and lymph node dissection.I am trained to perform Sentinel groin lymph node detection using radionucleotide scintography in combination with Patent Blue and Gamma probe detection, and more recently ICG detection of pelvic sentinel nodes laparoscopically.I understand pre-operative assessment, cardiopulmonary exercise testing, and the need for post-operative HDU for selected cases. Tailoring treatment given significant co-morbidities, the use of non-surgical treatments for cancers, and surgical access by muscle splitting incisions or Apronectomy.
