Case Manager
Current• Conduct comprehensive outreach services, assessments, and evaluations of client needs.• Develop a service plan and goals for clients.• Access critical resources and tools to improve individual well-being and emotional support advocacy.• Increase client knowledge of community programs and support to reduce social isolation.• Build and maintain the development of the collaborative service delivery and system navigation model with other older adult service agencies • Carry out case management functions including convening case conferences, liaising with community services. and other professionals including social services, justice and health.
