Transition Specialist
CurrentFrom June 2015-September 2016, I worked as a case worker for an Active Community Treatment team (ACT), where I worked with young adults who were undergoing severe mental illness. I would meet with each of my 20 clients approximately 2-3 times per week to connect them to services in the DC area, encourage them to take charge of their mental health and would provide emotional support as well. This job required excellent organizational, writing, and verbal skills in order to ensure that documentation/billing was done in an orderly fashion as well being able to advocate for clients who could not advocate for themselves.I was promoted in September 2016 to work as a Transition Specialist for the "Our Time: Healthy Transitions program". This program is designed to connect young adults who are experiencing mild mental health illness and to connect them to necessary services, such as employment programs, housing assistance and access to mental health agencies in D.C. This program was built from scratch where I was responsible for establishing partnerships with service providers in the D.C. community in order to build a case load and to then work with clients to connect them to the services they asked for.
