• Providing Senior Director-level oversight of the multiple national AHF Ryan White Medical Case Management (RWCM) Grant contracts/programs. Collaborates with Bureau Chiefs and Regional Directors to integrate and deliver care management services that promote AHF's North Star approach in the AHF Healthcare Centers (HCC) for growth, outreach, retention, improved medical and social outcomes, and patient satisfaction. • Responsible for leading processes as the subject matter expert to grow AHF census and Ryan White Care Management programs, i.e., from grant proposal development to the design and implementation of new and existing contracts, coordination of grantee monitoring audits, monitoring of fiscal performance and clinical outcome management, setting of growth/retention goals, policies and procedures and personnel accountability to standards. Analyzes, plans, and operationalizes care management services so that AHF goals and, contract guidelines and standards of care deliverables are met.• Establishes and maintains collaborative relationships with Ryan White Funders, AHF Bureau Chiefs, Regional Directors, AHF Contract Managers and Medical Directors and HCC staff so that an understanding of the Ryan White contract, services, and deliverables in their respective areas is known, supports and is integrated into the AHF workflows.

• Optimize the program managers to ongoing performing management by improving staff efficiency, prioritizing work assignments, enforcing policies and procedures; resolving staff issues; assessing training needs, monitoring program performance, contributing to staff retention, and mentoring Associate Directors and managers. • Develop quality management protocols and gap analysis to process, training development, and client satisfaction. Oversee financial operations, maintain budgets for all Ryan White programs, and work closely with the finance and grant department. Monitor and analyze care management productivity and performance reports, and identify trends, best practices, and challenges. • Creates, implements, reviews, and updates policies, standard operating procedures, and/or other stakeholders in the different contracts and other collaborative disciplines to assure collaborative care and communication while meeting patients ongoing chronic needs.

• Improved the Case Management program through monitoring, delegation, and ensuring efficient problem-solving. Operationalized all new processes and initiatives to improve patient outcomes and access to care and treatment. • Collaborated with the Contract Management Department to facilitate reporting and compliance. Participated in developing program budgets with Contract Program Managers for assigned Eligible Metropolitan Area (EMA)/Transitional Grant Area (TGA) and actively managed and ensured programs stayed within budget.• Created and implemented all Case Management policies and standard operating procedures and/or other stakeholders in the specific case management contract, including but not limited to the Grantor Standards of Care to ensure collaborative care and communication while meeting patients ongoing chronic case management needs.
