I have been working in the manufacturing industries for a while now. Especially FMCG types. And in all those industries, I have been working as an engineer in the maintenance and now production where by I get to the ground and take the spanner by the shaft.In all the areas i have worked in, I have found that ignorance is the most cause of machine failures despite the level at which the company is. However, as a leader, in the drivers seat, its not easy to change the culture of people, especially the mindset of those who learnt on the job but mechanisms can be designed to force them to change. Other times, putting perimeters also helps solve many of these ignorant related issues.After several analysis, I have also found that many times management agrees to carry out works, installations, bypasses on machines to just to have production temporarily but these temporary solutions end up becoming permanent and causing inefficiency and safety issues on the machines and operators. My advice is that let things be done right the first time. Planning early works best [my own views and experience]
