Interim HealthCare is the nation’s oldest health care franchise company and operates in 43 states. Our Agency (HHA) provides Home Health PT, OT, ST, MSW, SN, and CNA services, with Therapy services as the primary revenue driver. I manage the Therapy Staff, which includes PTs, OTs, STs, and MSWs, and I’m responsible for patient Case Management, Quality Improvement and Compliance, creating and implementing Clinical Protocols and new service lines, and overall Clinic Operations and Process Improvement. Results during my tenure include:• Increasing our Home Health Compare Star ratings from 3/3.5 stars to 4/4.5 stars for Patient Survey results (HHCAHPS) and Quality of Care, respectively• Selection by SCL Health as a Preferred Provider for the Comprehensive Care for Joint Replacement (CJR) bundle• Creating a CJR protocol for Intake, Clinicians and Managers to improve Timeliness of Start of Care, identification of significant comorbidities, and coordination of care with Orthopedic Navigator and other providers• Selection as a Preferred Provider by UCHealth – Longmont• Improving our Therapy Staff error-free medication reconciliation rate from 83% to 100%• Developing and implementing a process to transition the Functional Independence Measure (FIM) from paper to EMR, that increased our FIM completion rate to 100% for all Kaiser therapy patients• Creating and marketing a PT Dry Needling Program to make our Agency the first Interim franchise, and one of the state’s first Medicare Part A HHAs, to offer this intervention• Implementing a HIPAA-compliant secure text messaging system to improve Case Coordination• Educating our PTs to perform PT/INR finger-stick tests using point-of-care, in-home testing kits• Best ever top-line revenue and profit margin for our Agency

• When LifeCare at Home switched from paper to EMR, I quickly became proficient using Homecare Homebase.• Top-producing clinician across all disciplines, and the only therapist to complete over 40 patient visits in a week.• Although I had a high patient load, I provided exceptional quality care, and patients regularly called the office to voice their satisfaction.

• Initially, Amedisys had few orthopedic referrals in my territory in Longmont. I contacted a colleague at Front Range Orthopedics, and organized meetings and inservices with their staff. In less than a year, orthopedic referrals to Amedisys tripled in Longmont.• When I started at Amedisys, I had no experience in home health care, a very different beast than the outpatient sports medicine/orthopedics arena I was accustomed to. Amedisys needed me to get up to speed quickly to serve their rapidly growing census. I responded and accelerated my learning and assimilation into home health care, and in 2010 I became the top-producing clinician among all disciplines in our office, helping our office achieve its largest revenues and most profitable year to date

BCSM was part of Boulder Community Hospital and served the active community with sports specialists in 4 functional areas: Physician Clinics, Physical Therapy, Sports Science, and High School Outreach. As Physical Therapy Manager, I led a Physical Therapy staff of 25 clinicians, and I developed existing and new programs to grow the Center as a whole. • I partnered with local Physicians and produced a Lecture Series of a dozen Sports Medicine lectures per year. The referrals generated by the lectures was a driver in increasing revenues in all of BCSM’s functional areas, including a record $5 M in Physical Therapy charges in 2008.• I partnered with Eldora Mountain Resort and founded an advanced skiing program. I grew the program every year for 4 years, and tripled BCSM Physician, Physical Therapy, and Sports Science referrals from our skier segment.• BCSM’s High School Outreach program provided an Athletic Trainer to each of the 6 high schools in Boulder Valley School District. I spearheaded an initiative to get our in-house Physical Therapists out to the schools to partner and brainstorm with the trainers, coaches and athletes. This led to an unprecedented number of high school referrals to BCSM. • I created a PR campaign in which all Physical Therapists wrote articles for, or were interviewed by, local and national publications. These pieces drove referralls through publications such as the Boulder Daily Camera and nationally in the New York Times and Outside Magazine. • Besides BCSM’s own marketing budget, the hospital PR department was a potential resource for additional marketing, but BCSM had weak ties with them. I cultivated a relationship with the PR Director and enrolled his support for a variety of BCSM programs, and he agreed to run radio and print ads from his budget. This co-marketing enabled BCSM to more effectively roll out new programs, and we increased revenues and profit margins every year during my tenure.
