I have been appointed (May 2021) to a new role as Specialist Doctor in Learning Disability with Humber Teaching Foundation NHS Trust. The project has been commissioned by Hull CCG for 2 years and includes an evaluation of this position which is the first of it's kind in the UK. The purpose of the new role is to develop a new model of care delivery for improving the health outcomes and quality of life of individuals with a Profound and Multiple Learning Disability (PMLD) living in Hull. The role will include an individual MDT health support plan overseen by the post-holder from a medical perspective. This will be based on an assessment of all the disabilities and health needs of the individual by the MDT. To jointly develop holistic individual care plans and treatment plans with the MDT. Working with the family and a key health professional to form a small supportive team around the person with PMLD. To facilitate as part of the MDT that all individuals with a PMLD have clearly documented health information that can be quickly shared with others to help the delivery of appropriate care and support at all times. To support as part of the MDT the transition of clients with PMLD from children’s services to adult services enabling continuity of health care service provision. To increase knowledge and skill base of health care professionals and carers working with the PMLD population of Hull. I have recent experience in undergraduate medical education at Hull York Medical School and have the Postgraduate Certificate in Medical Education. I am the Honorary Secretary and Treasurer for Hull Medical Society which seeks to promote Medical Education and Camaraderie. The society holds regular educational events and provides opportunities for GPs and Hospital Doctors, Medical & Physician Associate Students and Retired Doctors to meet in a relaxed and informal environment for networking and support.
