CurrentAs a Board Certified Hospice Chaplain, Spiritual Care and Bereavement Coordinator at St. Croix Hospice, I ensure that care and comfort are tailored to your unique preferences. We will individualize your care to ensure that it is provided to you in a way that brings dignity, respect, and compassion - on your terms. We hear and honor the voices of our patients, families, and primary caregivers, as these voices are the substance of our work. A 300 patient Hospice specializing in medical care for terminally ill patients.• Provide emotional and spiritual support to hundreds of terminally ill patients and their loved ones at end-of-life. Also support team members.• Manage documentation of all pertinent chaplain activity and audited patient's charts.• Plan and lead annual memorial worship service for families of deceased patients.• Other activities include: Leading classes of: Coping with Stress, Effective Communication, and Grief Education continuing education for staff, facilitating grief support group meetings, developing bereavement plans of care for families of deceased patients, and assisting in completing health care directives.• Serve as chaplain liaison at bi-monthly patient roundtable meetings, providing detailed accounts of all patient spiritual needs and assessments.• Demonstrate knowledge of ethical issues/concerns and chart spiritual assessments; significant information and spiritual care interventions in patient's medical record.• Assess spiritual needs of patients and develop spiritual care component for interdisciplinary care plan, provide spiritual counsel.• Participate in seminars and supervisory sessions and complete written requirements for the Association of Professional Chaplains.• Participate in departmental responsibilities including assisting members with design, review and evaluation of spiritual care services, conduct worship services.• Ensure confidentiality with HIPPA guidelines.
