Able Care is on a mission to keep aging seniors at home and Able for as long as possible, working with at-risk providers to decrease readmissions and hospitalizations, while improving quality and risk adjustment scores. A tsunami of aging is about to hit our health systems. In the US alone there are 10,000 new seniors (65+) everyday. These seniors all suffer from Functional Decline, quite literally a decline in their ability to function independently at home. Outcomes of Functional Decline are already the leading drivers of hospital admissions and utilization. Providers have not been able to monitor the leading indicators for functional decline, the outcomes are only seen post event when they show up in claims data.
