My current role requires me to be dynamic and very good at problem solving. I work as a lone worker but also a valued member of the MDT. I provide care to patients in different community care settings with a range of acute and chronic health needs. I identify, implement, evaluate and reassess the care. I document this on paper and on the I.T system we use. This can vary from a simple dressing task, to a patient with complex needs that requires a holistic person centered care approach. I have many clinical skills that are used daily and are transferrable e.g. venepuncture, iv medication administration, wound and leg ulcer care, palliative and end of life care, catheterisation, bowel care, cannulation, central lines and many more. I work very closely with others from the MDT and am confident in communicating with these. I have responsibility for my own workload, time management and coordinating the team. I play an active role in our monthly palliative care meetings by discussing case management and being able to identify future input. I have completed nursing assessment reports, fast tracks and am able to produce accurate and legible documentation providing evidence for my clinical decision. I enjoy the mentoring of student nurses and ensure that the right learning environment is achieved. I also have a role in teaching and supporting peers, care givers and care receiver. I am very passionate about nursing and have high standards taking pride in my care and the team I work within. I thrive on being able to give the best care possible.

Oncology NurseBupa Feb 2014 - Sep 2014Manchester, United KingdomAs a oncology support nurse I provided clinical support, advice and care to those who had bupa health policy. I managed my own caseload and diary. I provided teaching to my peers about different oncology topics from what cancer was to what end of life care is. I had to carry out clinical assessments and authorise appropriate clinical treatments. Some of these were done on a quick turn over. I also Took part in being on call where you dealt with any new issues or queries. I supported those who had an oncology diagnosis and supported them and significant others through there cancer journey. I provided them clinical and non clinical support and advice, liaising with other professionals to deliver holistic care from a MDT approach. I provided care for people living beyond cancer and the challenges and anxieties this can cause. I provided health promotion and signposted to other organisations. This role was over the phone so excellent communication skills was essential. -
NursePennine Care Nhs Foundation Trust Feb 2013 - Feb 2014United KingdomAs a urgent community care nurse I worked within a team which included professionals from social services, physiotherapists and occupational therapists. The aim of the team was to prevent hospital admissions for anyone over the age of 18. Part of my role was to carry out urgent holistic assessments to assess the needs of the patient and to make a clinical decision, where these needs could be met. To provide the patient with appropriate care whether that be a community or a care Home setting. The assessments looked at the patient holistically and involved looking at this patient through the eyes of the other professionals. As part of this role I also supported nursing staff in a bupa nursing home who were commissioned to look after 14 nhs beds. As a band 6 I had to co ordinate a patients journey and had to manage there discharge from our service. It was in this role as a band 6 that gave me the opportunities to make team changes to allow our teams to work work affective by spectating the case load into teams that way we all knew who was doing what and there was ownership had by the professionals. Good communication skills and the ability to carry out complex holistic assessments was vital for this job. -

I provided care to those who were at the start of treatment and provided them with emotional support. I provided to those who were undergoing treatment and needed admission for symptom management and those that were end of life, which also involved in the management of their preferred place of care/ death. Which involved nursing assessments, discharge meetings and discussing with MDT. I administered iv therapy for eg chemotherapy and antibiotics. Also as a senior nurse I had overall responsibility of the running of the ward when on shift which involves the supporting of junior nurses and health care assistants. As well as this I worked on the oncology helpline which provided emergency oncology triage to patients and professionals and involved the assessing of that acute situation and whether admission was needed to rule out life threatening conditions. This assessment was carried out using algorithms but also clinical judgement. -
