Rn, Bsn, Pccn Clinical Placement Coordinator
CurrentCoordinate clinical aspects of care primarily focused on exploring and implementing best practices in order to continuously improve patient flow throughout the admission and discharge experience while maintaining optimal patient care, service excellence, and cost effectiveness. Coordinate interdisciplinary functions to assure appropriate clinical interventions, and quality improvement activities. Innovations to improve work procedures and methods which primarily result in some time and cost savings. The job duties affect direct patient care through the completion of tasks which allow others to provide direct care and may require provision of direct patient care, including admission assessment, invasive procedures, monitoring of patients during transport and during procedures with sedation. Assess bed status and resource availability to admit patients throughout the hospital and initiate a HOPE (Help Our Patients Everywhere) Alert as necessary to facilitate patient flow and make available beds for incoming patients. Role modeling and initiating teamwork and collaboration and professionalism across different departments hospital wide to facilitate optimal patient flow, patient satisfaction and service excellence. Coordinate the placement of patients throughout the facility for direct admits, in-house transfers, admissions or transfer from other facilities, and admits from the Emergency room, PACU and other outpatient departments. Coordinates admissions to assure maximum efficiency, decreased LOS in ED, and improve patient satisfaction. This includes resolving complaints, making adjustments or correcting mistakes about a selected service or capability. Job assign and monitor the flow of work for employees across work units. Provides input or completes performance reviews as requested. Work in close collaboration with the Nursing Supervisors, Medical Staff and Admitting Office on a routine basis.
