One of the most significant moments in my nursing career was achieving my RN licensure. Passing my board exam in July 2008, I have since been able to use my upgraded credentials in the field, as well as in my position as Clinical Coordinator. As needed, I still see some patients in the field, and this helps keep my clinical skills up to date. I truly enjoy the interaction with patients and families.

Restoration to the highest possible level of functioning in relation to health conditions, and advocacy with MD and other allied health professionals as appropriate, using the nursing process. Management of the Care Team to provide the best possible outcome for each patient.

Manager of field staff in home care setting. Coordinating care between nursing, therapy and support staff. Communication with patients, families and medical team, including doctors and labs. Staff education through in-service and precepting. Resource nurse for patients, staff and families. Research as needed for special projects or concerns. Resource person for current telemedicine and supply managers, as well as other staff as warranted.

The concept of telemedicine is innovative and exciting. The ability to assess data from patient to clinician remotely allows for quicker response to emergent issues and the potential for encouraging compliance with the clinician Plan of Care. During my tenure as Manager, this department utilized Honeywell HomMed in-home vital signs monitoring, which allows a patient or caregiver to complete vital signs measurements, as well as programmed basic triage information, by following simple prompts. At that time, data was transmitted via either telephone lines or by pager technology back to a secured web-accessed site, which could be reviewed by an LPN or RN. The nurse would call the patient if needed to clarify any concerns the vital signs provoked, and response could also include calling the physician to advise and allow changes to the Plan of Care, without requiring the patient to make a trip in to the MD (which might have been difficult due to the patient's condition.) Since that time, HomMeds have responded to the decreased use of pager technology by adding cellular technology for transmission purposes. Nightingale has also partnered with Intel to provide the Health Guide, and once again leading the way, Nightingale utilizes these guides to allow a patient to transmit response to daily therapy exercise remotely. Other duties of this department have included equipment management of HemoSense INRatio PT/INR machines, which allow point of service results of anticoagulation therapy labs. Nightingale technologies also include the MedX Infrared Therapy units, used to help improve circulation and relieve pain in patients with qualifying diagnoses, and the AMAC PERS unit, an advocacy piece that is placed in the home and allows the wearer to summon emergency help at the push of a button. Although I have taken on new duties since 07/07, I still have some involvement in this department

As Supply Manager, responsible for procuring medical supplies for patients in accordance with insurance guidelines and medical necessity for 6 states (Nightingale is now in 12 states.) Review of invoices for products purchase, and authorization for payment. Research advocate for more efficient products to use in the care of the patients of Nightingale. Audit of medical charts for proper medical orders to insure reimbursement. Although no longer the supply manager, I still slide back into this position as needed during times of staff transition or high volume, and to help locate specialty items as warranted. Attend inservices and demonstrations of products to see what is new in the industry, and to gain the knowledge needed to improve outcomes by use of specialized products for medically necessary care.
