Generative Space Task Force MemberWayne State University School Of Medicine Jun 2016 - Jan 2023UsThe concept of ‘generative space’ describes a place that both systemically and sustainably improves individual lives, organizational performance, and overall community wellbeing. The extreme outcome of a more generative space is ‘a place to flourish’. Learning how to achieve this is simple, practical, cost-effective, and low-tech. It is based upon demonstrated evidence from international research that has been field-tested and extensively peer-reviewed.Our mission ‘To work towards establishing Chicago as the world’s leading exemplar of the healthiest city for everyone living, working in, and visiting Chicago – based on producing experiences of generative space. In doing so, Chicago will become a learning laboratory for all other cities to learn from – and, in their doing so – to raise the global bar on improvements to individual health, organizational performance, and overall community well-being.’ -

University Goes Digital, Global Education Project--Co-Investigator, Eu Erasmus GrantSigmund Freud University Vienna Aug 2020 - Mar 2024Vienna, Vienna, AtBACKGROUND AND PURPOSEThe lockdown that forced teachers to make distance courses overnight both opened up avenues for those who had never used such tools before and showed the limits of an improvised shift to distance learning. This project will respond to that context with the design and experiment of a model of distance education that allows all students to develop competencies, whatever their background, and that makes the most of the possibilities of new technologies for a pedagogy that engages, motivates and supports all kinds of students. The purpose of the Project University Goes Digital is to improve digital skills of University teachers, to reinforce their capacity to respond the challenges Universities are facing during the COVID-19 pandemic or will face on future similar challenges. With active involvement of the lecturers and students from the beginning of the project, ATHENA will create, test and implement innovative digital practices, putting technologies in use to create new pedagogical approaches and achieve better learning and teaching experiences. The project seeks to foster cooperative learning environments, making them transformative and inclusive through the effective adoption of new technologies, such as e-learning, gaming platforms, virtual and augmented reality, systematically modelled to activate key competencies in digital learning. The project will create templates that lecturers can adopt andadapt to their classes, using different pedagogical approaches. It will be a toolkit that includes ebooks, videos, games, quizzes, AR and AI. The partnership is formed by 4 Universities (PT, FR, IT, AT), 1 technological partner (PT) and 5 associated partners: the Francophone University Agency (AUF) and others four from PT, Brazil, Cape Vert and Tunisia. The Universities partners will bring the know How of the courses areas and the capacity to implement the project. -

Oranizing Committee: Towards A Quieter AmericaAcoustics Research Council Sep 2011 - Sep 2023The years 2010–2012 mark a turning point in the history of noise policy in America. In this period, several official publications, published reports from leading research institutions, popular books, extensive media coverage and even a feature film provide substantial evidence that noise has become an issue of concern to Americans in sufficient numbers to create momentum for change. Public concern comes at an auspicious time for the United States: dramatic, cost-effective improvements must bemade to America’s health, education, productivity, global balance of trade, property values, law enforcement and quality of life. Modest changes to federal noise policies can help the United States achieve these goals.Towards a Quieter America is a seven-phase program launched in 2010 to educate America’s leaders about the value and importance of the twenty-seven recommendations in the 2010 National Academy of Engineering report,Technology for a Quieter America and the 2011 World Health Organization report, Burden of Disease From Environmental Noise.Together with other recent reports, these documents provide a roadmap for change.Towards a Quieter America is endorsed by the National Research Council Transportation Research Board/ADC40, the National Hearing Conservation Association, the Institute of Noise Control Engineering, ASHRAE TC2.6, and ANSI S12 Work Groups 41 & 44.Towards a Quieter America has three goals:A. Change perspectives through continuous, bi-partisan constituency-building;B. Organize and mobilize a large and powerful constituency representing government, science, healthcare, law, engineering, education and industry;C. Continue achieving science-based, incremental successes while avoiding confrontation.

Nihr Funded Global Respiratory Health Research Unit - Grant ReviwerWayne State University Oct 2017 - Jan 2018Detroit, Mi, UsA key aim of RESPIRE is to develop the capacity of researchers in respiratory health in the partner countries: Bangladesh, India, Pakistan, and Malaysia. We request please that this consideration be uppermost in the minds of reviewers, in the sense of seeking and fostering potential for a great project, rather than necessarily being a great proposal at this stage. We especially seek recommendations on how to transform proposals with potential into robust projects. The timescale in which projects have been developed has been unavoidably very tight (several weeks rather than several months) and so project leads, candidate researchers, and academic staff supporting them to develop initial concepts into proposals have been working against the clock. -

Keynote, Japanese Surgical Society Annual ConferenceWayne State University Apr 2017 - Apr 2017Detroit, Mi, UsDesigning High-Reliability Surgical Health CareDespite serious and widespread efforts to improve the quality of health care, many patients still suffer preventable harm every day. Hospitals find improvement difficult to sustain, and they suffer “project fatigue” because so many problems need attention. No hospitals or health systems have achieved consistent excellence throughout their institutions. High-reliability science is the study of organizations in industries like commercial aviation and nuclear power that operate under hazardous conditions while maintaining safety levels that are far better than those of health care. A high reliability organization (HRO) is an organization that has succeeded in avoiding catastrophes in an environment where normal accidents can be expected due to risk factors and complexity. Japanese surgical leadership should thinks more strategically about developing and funding a bold research agenda in Japan which aims to expand operative assessment beyond patient factors and the technical skills of the surgeon/anesthesiologist; to extend assessment of technical skills beyond bench models to the operating theater and its equipment; to provide a basis for assessing interventions; and to provide a deeper understanding of surgical outcomes. Research into surgical outcomes has primarily focused on the role of patient pathophysiological risk factors and on the skills of the individual surgeon/anesthesia provider. However, this approach neglects a wide range of human/environmental factors that have been found to be of important in achieving safe, high-quality performance in other high-risk environments. The outcome of surgery is also dependent on the quality of care received throughout the patient’s stay in hospital, the reliability and suitability of the surgical equipment and physical settings, and the performance of a considerable number of surgical team members, all of whom are influenced by the environment in which they work. -

Yet there are many tensions and unintended consequences to be considered in adopting innovations – such as the balance between productivity and patient - centered care. The speakers will engage in a provocative conversation about what these tensions are and key considerations and interventions needed to help gain clarity in designing for innovation that is fit for purpose and meets future population health needs.

Keynote And Workshop On Critical Drivers For Successful Leadership In Quality Improvement Oct 7/15155Th Middle East Patient Safety Conference Oct 2015 - Oct 2015Health care leaders concerned about quality must build systems, often from scratch, that are capable of delivering quality care or enabling external partners to improve quality within their organizations. In tackling this challenge, these leaders, like all leaders, must rally their staff in pursuit of a common goal. Adverse health care events remain a leading cause of death and injury around the world, even though evidence-based methods are often available that could prevent these outcomes. Meanwhile, there is a growing recognition of the importance of culture in preventing such errors and harm.

Plenary Talk: Redesigning Hospital Alarms For Patient Safety: Alarmed And Potentially DangerousThe 11Th International Conference On Noise ( Icben) Jun 2014 - Jun 2014Alarm safety is one of healthcare’s most high-profile and intractable problems. A phenomenon known as “alarm fatigue”, including limited capacity to identify and prioritize alarm signals, has led to delayed or failed alarm responses and deliberate alarm de-activations. Alarm fatigue has been implicated in patient deaths, some highly publicized. There is a growing acknowledgement that this acoustical problem results in hundreds of identified patient deaths and thousands of injuries. The healthcare facilities industry, however, has been slow to recognize that “alarm fatigue” is in part a facility design issue.

Co FounderTelluride International Patient Safety Roundtable Jul 2005 - Jul 2009Co-founder and leader of Telluride International Patient Safety Roundtable; Patient safety has emerged as a global concern in the provision of quality health care, and yet, to date, few medical schools have created and/or implemented patient safety curricula. The purpose of this article is to introduce readers to one model of a patient safety undergraduate medical curriculum, as designed by a group of experts attending an annual interdisciplinary roundtable assembled for this purpose. Summary: The Annual Telluride Interdisciplinary Roundtable met in 2005 and 2006 to design what it considered to be a comprehensive patient safety curriculum for medical students. Invited members included stakeholders from a variety of fields, including health care providers, senior health care administration, students, residents, patient advocacy leaders, and curriculum development/assessment experts. The group developed a list of general curricular principles, followed by 11 specific elements felt to be essential to an effective patient safety curriculum for undergraduate medical education students. It also identified a number of challenges to implementing such a curriculum. Conclusions: A patient safety curriculum, developed by a group of experts for an undergraduate medical education population, was successfully developed over a two-year period of time. Future meetings of the Telluride Roundtable group have centered on evaluation and refinement of these curricular elements as pilots occur in a number of medical schools, and new curricular ideas continue to be developed. Continued interprofessional dialogue and collaborative research will enable the development and im- plementation of a standardized longitudinal patient safety student curriculum.

Human Factors Analysis Of Pediatric Blood & Marrow Transplantation Unit--DukeAptima, Inc./ Jan 2007 - May 2007The Pediatric Blood & Marrow Transplantation Unit (PBMTU) of Duke Hospital is at the vanguard of clinical research concerning several severe pediatric maladies. The families of PBMTU patients respect the skills of the team that cares for their children, and they appreciate their support through the difficult times. The unit has experienced several sentinel events, the turnover of nursing staff has grown, and there is debate that some practices may conflict with prescribed procedures. In the face of these and other challenges, the PBMTU retained Aptima to conduct a human factors analysis of its operations. A human factors analysis addresses an organization as a complex socio-technical system; it identifies the stress points system and redesigns it to prevent errors from occurring, trap them while they are inconsequential, or mitigate their effects. More positively, redesign of systems helps people and machines do the right thing with less effort.Aptima engaged a multi-disciplinary team of staff and consultants in this work: specialists in clinical safety, decision making, teamwork, engineering, computational modeling, psycho-social services, long-term critical care. Members of this team collected data through document reviews and more than 50 individual interviews, focused group interviews, and direct observations of procedures. The participants were mainly stakeholders in the PBMTU – including nurses, physicians, psycho-social specialists, administrators, and parents – and, to a lesser extent, members of the larger Duke University Hospital of which it is a part. All members of the team participated in data analysis and contributed to this report of findings, recommendations for action, potential risks, and measures of process and effects. In addition, we developed a computational model of work during the line change period in order to safely, rapidly test the effects on workload of changes to staffing and procedures.

Teaching Life Support Competencies In Health Care –Assessment Of Current Practice And RecommendationThe University Of Birmingham, Edgbaston, Consultant 2006 - 2007Objectives: The study sought: 1) to survey the type of courses teaching adult life support and pre cardiac arrest management provided by UK hospitals and medical schools and required by professional bodies; 2) to describe both the curriculum content and applied teaching methods used the most popular in-hospital adult life support courses; and 3) to examine the fidelity of adult life support courses and their implementation in England; 4) to stimulate policy makers in other countries that are examining the utility and effectiveness of life support coursesMethodology: We surveyed the prevalence, distribution and impact of the resuscitation training population among UK health trusts. We conducted semi-structured telephone interviews with a randomised sample (n=30), and then a detailed thematic analysis of written and online course materials. Results: We found a large number of both providers and types of life support courses for health care professions, the public and special groups such as divers. All but one of the thirty English NHS Hospitals surveyed provided adult life support courses. The Resuscitation Council UK Advanced Life Support (ALS) course was run by a majority (n=20) of the acute hospital trusts surveyed. Many hospitals ran ‘in-house’ courses alongside or in addition to the national (franchised) courses. Our survey revealed that courses aimed at the recognition and management of pre-arrest deterioration are now widely taught in hospitals (n=25). The survey of UK medical schools (n=25) revealed that while life support training was provided at all sites, courses varied and those aimed at the recognition and management of acutely ill patients were provided by only six sites. Only three professional colleges surveyed (n=16) required members to provide evidence of life support skill competence. More study is needed to assess the impact of these courses on clinical care and patient outcomes.
