A variety of models and support systems are embraced among our members, including: clinics fully supported by private donations and grant funds; clinics supported through combined hospital system and government resources; and, hospital-affiliated clinics. The common thread across all clinics is our commitment to accessible high-quality health care for high risk populations. As the Affordable Care Act begins to change the landscape of health care we are committed to assisting our patients in signing up for either BadgerCare or an affordable Health Care Marketplace Plan. At the same time, our clinics will continue to serve people who struggle to afford copays, deductibles and medication costs or who fall through the cracks with no other options for care.As a coalition, our clinics are committed to:Share resources, including medications, specialty care, health education, medical supplies and equipment, grant writing, information technology, dental and mental health services.Support each other through information sharing, networking and expanding access points for our patients.Advocate for our uninsured and underinsured patients, not only giving a voice to those those who would otherwise be unheard, but working to improve health care policy that affects these patients.Work to reform the health care system to provide access to care for all, and by practicing what we preach.Promote and educate the community about the scope of services provided by free and community clinics.Contact me at phoffm2298@aol.com for more information about FC3
Clinical Pharmacist, Director Of Medication Access
I am proud to be providing over $4.5 Mil worth of medications to uninsured patients via Bread of Healing and the Free and Community Clinic Collaborative (FC3). Contact me if you are interested in learning more.OUR MISSIONThe Bread of Healing Clinic is a neighborhood-based clinic for individuals with financial or other barriers to quality health care. We commit ourselves to respect, love, and learn from the people we serve believing that the atmosphere we create reflects Jesus’ healing ministry.OUR VALUESLoveWe demonstrate God’s love for all by sharing hope, faith, and a generosity of spirit with those we serve, one person at a time.HealingWe believe that God calls us to healing in every arena of human life.HumilityWe are committed to the belief that we are all children of God, each with our own and differing fits. None of us has all the answers, and we look to each other to learn.InclusionWe serve all, regardless of individual faith, and we welcome all who wish to contribute their time and talents in service to this mission.Social JusticeWe work together to ensure that every person has access to the means to achieve health, recognizing that physical, emotional, social and spiritual health are intertwined and cannot be separated.EducationWe recognize the need and commit ourselves to the task of educating the next generation of health care professionals and paraprofessionals in an environment where the whole person is served and where patients have been historically underserved.PrayerWe rely on God as the source of our power, and prayer as our communication with God.A recent study estimated that the care provided to 2000 uninsured patients in that year saved $4.16 to $4.64 million in health and dental care costs in Milwaukee at large at a cost of $1.35 million. The major sources of these savings were avoidable emergency department use, reduced hospitalizations, increased patient earnings and improved quality of life.
Co Chair, Milwaukee Adult Immunization Workgroup
The Adult Immunization Work Group was Chartered on February 16, 2018. It is a collaborative group Convened by: Immunize Milwaukee/Milwaukee Health Department, Milwaukee Healthcare Partnership, Bread of Healing Clinic and a variety of other Community Groups.Access to adult immunizations for low-income Milwaukee County residents has been identified as an area for improvement by many of the County’s free and community clinics, FQHCs and health systems. The purpose of the adult immunization workgroup is to understand the scope of the problem, identify potential barriers to access to immunization, and to develop strategies to improve access and increase rates of immunizations for low-income (Medicaid and uninsured) adults living in Milwaukee County. The desired outcome of this workgroup is to increase the immunization rates for low-income adults in Milwaukee County for the flu, Tdap and pneumococcal vaccines. This will be measured by tracking the rates of immunization for this population (using immunization rates in Milwaukee for Medicaid and uninsured as a proxy for low-income) as compared to Milwaukee County’s overall immunization rates. Data for this comparison is provided by the Wisconsin Immunization Registry. Given that community clinics treat only a portion of the target populations, a >1% increase in immunization rates for Medicaid and the uninsured is the desired outcome.
