My role as a consultant is to ensure medication management follows best practices and to provide clinical and analytical expertise to guide medication management and patient monitoring. Our team partners with organizations to provide pharmaceutical services in the long-term care industry. I work with our clients to provide formulary management, improve transitional care and accuracy of medication reconciliation, develop policy & procedure involved in medication prescribing and monitoring, reduce potential for medication errors through education and EHR safeguards, reduce medication regimen complexity and guide deprescribing efforts in accordance with current clinical and regulatory guidelines. I provide medication expertise and analytics to various initiatives within our partner facilities aimed at improving patient care. My goal is to promote the well being and mental health of our patients, their family members and caregivers as they progress through the aging process and require higher standards of care.

As a Staff Pharmacist at our location in Syracuse, NY area I directly provided pharmaceutical services and medications to our customer base across the CNY area. The pharmacy is responsible for dispensing medication accurately and timely, with a focus on safeguarding our patients against harmful drug interactions, dosing errors and potential adverse effects from polypharmacy. In addition to dispensing, I provided immunization clinics for assisted living facilities and developed in-service educational programs to improve medication usage and management internally and for our customers. Skills developed during this role include ADS inventory management, use of technology and robotics to improve dispensing process and prescription accuracy, EHR / formulary integration with dispensing software and protocols, and policy & procedure development aimed to improve quality and accuracy of services provided.
