Utilized CRISP data to target primary care providers who cared for potential WISH clients for the program’s marketing efforts.• Adjusted program strategy based on continuous analysis of program data, driving a two-fold increase in newly active clients from Quarter 4 of 2018 to Quarter 1 of 2019. • Worked with health systems, community organizations, and county emergency management services to develop and implement processes for reducing unnecessary hospitalizations by successfully transitioning patients from acute care setting to community-based services provided by the WISH program. Developed education materials for clients, created workflow processes to facilitate transitions, and assured continuous flow of data between WISH and its community partners to facilitate the joint management of clients.
