Responsible for overall performance of home visiting program employing community health workers to address the social determinants of health for seniors in Montgomery County and thereby reduce avoidable hospital utilization and total health care costs. • Conducted on-going analysis/evaluation and reported on program performance against agreed upon metrics, including reductions in total cost of care, improved quality of care, and increased program enrollment. Reported in writing and verbally to contract monitors and the board of the six-hospital collaborative funding the program.• Led/managed internal team of 21 staff, including RN case managers, community health workers, outreach specialists, a program manager, office staff and a data analyst. • Utilized pre-post reports from state’s health information exchange, CRISP, to identify reductions in total cost of care for WISH patients. Utilized CRISP data to target primary care providers who cared for potential WISH clients for the program’s marketing efforts.• Adjusted program strategy based on continuous analysis of program data, driving a two-fold increase in newly active clients from Quarter 4 of 2018 to Quarter 1 of 2019. • Worked with health systems, community organizations, and county emergency management services to develop and implement processes for reducing unnecessary hospitalizations by successfully transitioning patients from acute care setting to community-based services provided by the WISH program.
