A Line Operations Safety Audit (LOSA) is an observational program for collecting safety-related data during normal operations, built around the Threat and Error Management concept. Gathered data is used to develop and implement measures to manage and improve the safety of the operation. Etihad conducted a 2-month LOSA observation campaign in 2015.- Established trust and good rapport with observed crew and assured them of anonymity and the no reprisal policy of the campaign in order to ensure authentic, usual behaviour in the cockpit- Identified and assessed threats to the operation and monitored: the crew’s threat mitigation strategy, any human error caused as a result of the threat, and the crew’s performance in handling such error made- De-identified, coded, and entered all observation notes into the LOSA database

FOX-TEK developed non-intrusive pipeline monitoring systems for the oil and gas industry to track the thinning of pipelines due to corrosion.
