About Philip O Reilly
I am currently working as a Medical Social Worker with the HSE Carlow/Kilkenny ICPOP team. I am also a Mentor on the Apprenticeship in Social Work and the Assisted Decision Making (Capacity) Act 2015 Mentorship Programmes. I recently completed a NFQ Level 9 Safeguarding Adults at Risk of Abuse module in Trinity College Dublin. In 2022, I completed a NFQ Level 9 Certificate in Principles of Integrated Care in Munster Tecnological University. In 2021, I qualified as a Social Worker from University College Cork and received a 1.1 grade in the Masters in Social Work programme. I am also a qualified teacher and am a member of the Teaching Council.I worked with ALONE for three years in the office of the CEO developing new services and partnerships across the country. I have worked in Senior Management roles in the Community and Voluntary sector for over fifteen years. ALONE is a dynamic organisation that provides Housing with Support, Support Coordination, Befriending and campaigns for change. Prior to working in ALONE, I was the National Co-ordinator of Services for Older People with Respond Housing for 10 years. Respond is one of the largest approved housing bodies in the state. I was a member of the Respond National Senior Management team and was heavily involved in the delivery of quality services to tenants throughout the country. I am an active and vocal advocate for the rights of older people and have a particular interest in the whole area of loneliness and social isolation among older people in society today. I also have an interest in the Safeguarding of Vulnerable Adults and sit on the HSE Safeguarding Steering Group in CHO 5. I am a regular contributor on the Paul Brophy show on Kilkenny Community Radio as well as regular appearances on KCLR96FM. Prior to joining Respond, I worked as an Advocacy Officer with Age Action in the south east region. I am also a member of the Teaching Council of Ireland and regularly lecture on Housing, Community Studies, Health Skills and Care of the Older Person courses. I have been invited as a guest lecturer on Social Science, Social Policy and Planning courses in University College Cork. I have written articles which were published in the Chartered Institute of Housing Journal and the Irish Journal of Applied Social Studies. In my spare time I coach underage mini rugby with Kilkenny RFC and hurling with O' Loughlin Gaels GAA Club.

CurrentIntegrated Care Programme for Older Persons (ICPOP) is a multi-faceted approach to designing and delivering integrated care across local communities and hospitals between providers, users and carers. The concept requires a complete change in how health and social care is delivered nationwide, with a strong focus on patient experience and quality of care. Health and social care systems recognise that sustainability strategies lie in a population based health approach. The key cohort of this approach is older persons. At the essence of such change is a need to transition from an acute, episodic care to longitudinal, coordinated and integrated care models.

In ALONE we work with older people who are socially isolated, homeless, living in poverty or crisis and we support them to age at home.We provide direct support to hundreds of older people every week through our four main services – Support Coordination, Befriending, Housing with Support, and Campaigns for Change.In ALONE we do not duplicate services provided by other organisations, but work in partnership to ensure the best outcome for the older person in need.
