➤ Managing a 21-bed Men's Extended Care Program, providing care coordination for a large caseload and providing clinical case management, as well as providing IT training to staff and clients.➤ Conducting individual one-on-one counseling sessions and facilitating group counseling, using non-confrontational motivational strategies to promote engagement and retention in treatment, along with adherence to rules and positive change.➤ Collaborating and supporting the patient and their family in developing their individual Recovery Plan and Continuing Recovery Plan, acting as a resource in identifying the ideal treatment methods along with other recovery support services to meet their recovery goals.➤ Providing person-centered support and recovery coaching, including designing activities for family members such as family conferences and "Family Nights", as well as coordinating care with outside agencies as needed by the individual.➤ Developing and presenting educational materials and lectures on a variety of recovery-based topics including substance use disorders, recovery planning, relapse prevention, living skills, and other relevant topics.➤ Acting as an advocate to help the men obtain all needed support services during the course of treatment and following discharge, including helping to find employment and advocating for them in various legal situations.➤ Providing progress and other reports to those involved in the individual's treatment, including DCFS, DHS, TASC, courts, probation, and employers, as well as providing utilization review updates to insurance or other managed care providers.➤ Forging relationships with other agencies in order to secure affordable services to meet clients' needs, while also providing outreach to local companies who now hire exclusively from Stepping Stones.

➤ Completed initial screenings to determine if incoming clients met the requirements for the American Society of Medicine's levels 1 through 3.7 offered by the program, reviewing provided medical information to ensure a person's fitness for treatment and the need for a doctor's referral.➤ Conducted person-centered, strength-based assessments for substance use disorders in order to provide accurate diagnoses based on DSM-5 and ASAM criteria, as well as to determine an appropriate level of care, addressing immediate/crisis medical or mental health issues.➤ Worked in a collaborative manner with the individual and their family to complete an Assessment Summary, identifying their needs, recovery goals, and other information, while establishing a therapeutic alliance.➤ Coordinated admissions, transfers, and discharges, including completing the discharge process for individuals who did not begin treatment as scheduled, as well as contacted third-party payers when necessary to ensure admission and benefits availability.➤ Reviewed delayed admissions with the Director of Admissions & Outpatient Services, while coordinating with the RN on staff or the Medical Director to address mental issues that might preclude individuals from treatment.➤ Presented new residents to the unit when possible and provided an introduction to the residential staff, as well as communicated all admissions with Client Accounts and coordinated with the Program Directors as needed.

➤ Supporting the MA Executive Office of Health & Human Services and law enforcement agencies in the Commonwealth of MA, helping to safely transition runaways in police custody to local emergency service programs for assessment.➤ Procuring transportation for the runaways to a residential setting once their assessment is complete and court is not in session, as well as advocating for the minors when appropriate and maintaining cooperative working relationships with area service providers for better outcomes.➤ Providing information and referral services for MEMA, FEMA, CDC, and other government agencies during states of emergencies.➤ Handling many previous roles in the organization over the years, including database management, outbound campaigns, and training newly hired employees, as well as connecting individuals and organizations to volunteer opportunities.➤ Responding to each inquiry, assessing the needs of the individual and listening in a professional, non-judgmental, and culturally appropriate manner, while providing accurate information and enabling the inquirer to choose the most appropriate resource.➤ Connecting callers to information about critical health and human services available in their community, as well as serving as a resource for finding government benefits and services, nonprofit organizations, support groups, donation programs.
