My name is Phyllis Middleton; I would like to take this time to introduce our PPEC Pediatric Extended Care facility. Tender Care Medical Services of St Johns County, Inc., Our facility is a licensed, non-residential, day treatment program offering services for medically complex and technologically dependent children (from birth to 21 years of age). We operate up to 12 hours per day (including transportation), M - S. Our services include, but are not limited to, providing individualized skilled nursing care by a team of dedicated pediatric professionals, physical, occupational, speech and respiratory therapy, and, we provide transportation to and from our facility (servicing St. Johns, Putnam, Flagler, and Clay counties). Our PPEC center is designed to minimize exacerbations of chronic medical conditions and to decrease the need for re-hospitalization. It also provides an alternative health care choice for the families of children with special needs, by allowing the children to socialize with other children instead of being homebound.We also provide services to children, who are enrolled in school, during school closings and most holidays and over the Summer Break or after school care. This provides continuum of care while school is not in session.Our Mission is dedicated to providing the highest quality of medical care while also providing rehabilitative therapies in order to increase the quality of life for these children.
