Case Manager
CurrentNCP Choices Programs• Providing assessment and ongoing assessment of a client’s need for Case Management services• sourcing, interviewing, and onboarding.• Obtaining input from the person /providers/significant others about the service delivery process and seeking information from anyone to obtain needed services/supports on behalf of the person.• Informing clients about benefits, community resources, and services• Assisting clients with accessing community resources and services• Arranging for the receipt of benefit and services to include financial, medical, legal, educational, vocational, psychological/psychiatric, transportation, residential, social, and recreational, as need and indicated.• Providing ongoing assessment of each client’s response to treatment; informing significant other about the person’s progress and effect of case management efforts• Create updates and ensure completion of plans according to contract deadlines.• Attend scheduled supervision meetings to discuss participant’s progress.• Outreach to referrals who fail to show for appointments. • Maintains complete accurate and timely case notes and related records in the manual and electronic system.• Conduct an assessment with NCP Customers, which includes employment and support service needs.• Complete and develop an Individualized Employment Plan (IEP) • Inputting Customers in Texas Workforce Information System of Texas (TWIST)
