I worked in an MSF funded welfare home, mainly working with residents above the ages of 50 years old. I am part of the case management team. Each resident has an Individualised Care Plan in which includes holistic approach comprises of medical, social, financial, engagement and independence level. I worked with external agencies such as MSF, employers, nursing homes, FSC and SSO and more to ensure optimum care of resident. My caseload is approximately around 60 cases, but I do know all 130 residents within the Home. I also ensure timely submission of monthly reports to MSF and achieve KPI for case management team as set by MSF.
