Assistant Care Coordinator at NUHS.I have always been passionate in working in the healthcare sector and with various age groups. I have gained experience by interning at NTUC Health Active Ageing center and involving in various health projects with industry partners such as AWWA and National Heritage Board to create and implement a health programme in community homes.As a care coordinator, I am tasked to help individuals to better manage their chronic diseases and enhance patients' understanding of social and physical activities, lifestyle management, and social prescribing. Currently I'm taking my Specialist Diploma in Health and Wellness Coaching in Republic Polytechnic under the Work Study Program. I aim to upskill myself with the relevant skills and knowledge and apply it into my role as a health coach.
