Job duties include but not limited to: Provide direct care to residents, oversee CNA's, monitor vital signs, administer medications via oral, subcutaneous, IVPB, intradermal, inhalation, peg tube, NG tube and topical routes. Also, documentation, chart checks, assessments, and communicating changes in residents behavior or health status to physicians, and family.

Clinical Care Coordinator which include transitioning patients from hospital to home. Upon discharge reaching out to patients telephonically to educate on diagnosis and medication. Ensuring all medical equipment and home health if needed is in the home or someone has reached out to the patient. In addition, made certain that hospital follow ups were in place along with transportation if needed. Also, assisted with many other resources that patients needed.
