Case Manager
CurrentSubstance Abuse Prevention Center (SAPC) service provider- Manage caseload within inpatient and Co-occurring Integrative Network (COIN) program- Link client to relevant community resources, public social services, transitional housing resources, and benefit applications to determine eligibility for DMC Medi-Cal insurance coverage- Coordinate case consults with external service entities including: probation, court diversion providers, and DCFS to aid in obtaining all court minute orders and scheduling supervised visits- Prepare client progress letters, standard court reports, and safe exit planning- Assist with weekly group facilitation of substance abuse disorder patient education utilizing evidence-based practice curriculum depending on assignment or client need
