Forevermore was a start up company created to participate in the PASSE managed care medical program in Arkansas. Forevermore was successful during Phase I of this project which consisted of Care Coordination for assigned members with significant behavioral health or developmentally disabled diagnosis. We hired Care Coordinators across the state to service our members. As the program moved to Phase II, managed care, Forevercare assessed there was not sufficient time for a successful implementation and decided not to move forward. My role then shifted to working with Arkansas DHS on a transition plan to move our members to other PASSE companies. I was also responsible for working with our staff and board to close down the company.
