As an Intake Specialist at Thresholds - a community mental health agency - I met with members in long-term rehabilitation centers, nursing facilities, public places, psychiatric hospitals, and Thresholds locations to perform mental health assessments using the IM+CANS, LOCUS, and MOCA. I also performed resident reviews, and assessed members through the Williams/Colbert, Housing, and Front Door Diversion programs. The assessments covered complete life histories of education, employment, ADLs/IADLs, living situations, family life and relationships, legal, substance use and treatment, mental health, mental health hospitalizations and treatment, physical health and treatment, trauma, risk, medication, strengths, and finances. I completed care plans, risk mitigation plans, and comprehensive service plans, as well as risk assessments and service notes. I collected collateral information from guardians, family, medical and mental health providers, and friends.

Dance/Movement Therapist at a behavioral health hospital. Provided group and one-on-one services for multiple populations in outpatient, inpatient, and residential settings. Patients included children, adolescents, adults, and older adults, and include populations that experience chemical dependency and other addictions, eating disorders, school anxiety/school refusal, obsessive compulsive disorder, self injury, anxiety/depressive disorders, psychotic disorders, trauma-related disorders including PTSD, and dementias.
