• Coordinate the delivery of effective relationship based care of the orthopedic surgery patient based on the physician and nursing plans of care throughout the continuum• Complete rounds on all postoperative patients in house• Actively engage the patient and family/caregivers in care discussions from the preoperative surgical appointment to the post-discharge care appointment• Facilitates care for both pre and post operative patients• Coordinate solutions for clinical issues, questions and problems• Facilitate and present preoperative joint camp meetings• Recognize the needs of the patient population and lead clinical program improvement and changes• Coordinate with the hospital’s rehabilitation department and chair of orthopedic surgery on what education sessions may be of value to patients and the community.• Teach or participate in preoperative classes as needed• Develop and deliver in-services/training on new and existing procedures, protocols, and equipment on a regularly planned basis• Deliver pre and postoperative education to patients, support persons and/or staff • Perform outreach to all internal and external customers, including doctors, patients, and the community. • Serve as a consultant to peers & others regarding complex clinical conditions/cases • Act as a resource at the unit, department and, when needed, system level• Develop and lead initiatives to promote continuous readiness with Regulatory and Specialty Standards. As required, secure or maintain certain accreditation by adhering to requirements and implementing new processes as required by the American College of Surgeons• Participate in models of care programs implemented by hospital or system• Initiate and participate on hospital teams and committees to improve protocols and the overall effectiveness – clinical and cost- of programs• Identify areas for quality improvement and implement associated action plans; working cross functionally
