Answer Medicaid recipient related phone calls and provide information regarding covered services, health plan choices, and referrals to community and government agencies. Handle all calls with respect and dignity and follow HIPAA guidelines to ensure confidentiality is maintained. Log all complaints, health plan enrollments and interactions into appropriate database system.

Determined eligibility for the federally funded Energy Assistance Program by evaluating applications and calculating household income. Assisted clients both in person and via phone to complete the application process. Worked with energy companies to avoid shut-offs and set up cold weather protection.

Determined eligibility for a government subsidized health insurance program by processing new applications and yearly renewals. Worked with a variety of databases to access information regarding eligibility.

Provided case management, referrals, resources and follow-up for elderly persons living in Northeast, Southeast, and North Minneapolis. Provided intake for and served as backup staff for Friendship Center Adult Day Program, marketed Senior Programs to the senior community and service providers, collaborated with neighborhood organizations to assist seniors with Medicare Part D and facilitate access to services.
