Care Manager Supervisor
Current• Completes audits and other quality assurance/improvement duties of care manager caseloads to determine areas of improvement or focus and to monitor for upcoming end dates for closures or notifications to be sent. • Review, approve, and submit Over Cost Containment requests for PAR approvals to the state. Submits and monitors cases for review when meeting certain cost thresholds.• Conducts bi-weekly team and individual care member meetings to identify areas of focus, relay management/HCPF communications, field team questions, and other guidance as needed regarding member-specific questions or situations.• Represents organization at PDPPC CDASS monthly calls.• Assists team with managing caseloads via completing coverage visits, 6 month and quarterly phone calls to members, and other follow-up as needed.• Communicates with and relays important information between department managers and care managers as needed. • Responsible for creating and maintaining a positive team work culture. • Responds to formal and informal member grievances and engages in resolution processes as needed. • Involved in creating a training schedule and supervising training activities for new case managers. • Monitoring open caseloads for member or provider issues and questions; Forwarding such requests to the covering case manager and providing follow-up of requests to ensure completion and satisfaction.
