Sickle Cell Foundation of Minnesota (SickleCellMN) was founded in 2012 and became a 501c3 non profit in 2015. We are comprised of passionate patients, caregivers, sickle cell trait carriers, medical providers, and other stakeholders and individuals who are concerned with the outcomes of those living with sickle cell disease. Our mission is to improve the lives of individuals and communities affected by sickle cell disease through research, education, advocacy, equitable access to knowledgeable and compassionate medical care, and community empowerment. The Sickle Cell Patient Coordinator & Health Advocate works with patients and their families who are impacted by Sickle Cell Disease (SCD), providing culturally appropriate health education and information, providing sound guidance on health behaviors, advocating for patients’ individual and community health needs, providing medication management support, plan of care support, and resources and referrals to direct services.Skilled execution of this role also includes: * Support with medical treatment plans, addressing barriers and identifying unique patient/family needs. * Assisting patients/families with navigating various system, including healthcare and education. * Providing individualized supportive coaching that optimize the health and function of individuals living with sickle cell disease.* Transition preparation, readiness, and support throughout the transition process.

* Develop and maintain efforts that ensure the success of the organization's Sickle Cell Program. Facilitate presentations to large organizations, community groups, high school students, and individuals, with a strategic aim to increase education and awareness and establish long-term relationships and responses within the African/African-American community* Work with key internal and external contacts to establish, build, and maintain minority donor base and develop new sponsor accounts

 Provide effective classroom and field instruction for newly-hired Collections staff, perform annual competency assessments, and participate in the delivery of organization-wide training, as requested  Effectively teach and practice systematic and standardized processes in accordance with company QRA guidelines and FDA regulations Assisted in the designing of training materials that integrate content and course enhancements to close identified knowledge gaps, address quality issues, and promote increased consistency in service practices Actively participate in establishing, measuring, and achieving departmental and personal performance goals

The Product Acquisitions Coordinator is charged with managing and expanding product donors and donations, including individual and corporate donations. Overall, the Coordinator's goal is to establish deeper relationships with existing partners as well as establish and grow future partnerships that will increase donations and assist organizational sustainability that helps to ensure the mission of The Food Group moves forward.
