Transforming Singapore's healthcare system, a step at a time. Currently supporting the Institutional Care Collaborations (ICC) division of NUHS Regional Health System Office, Rae contributes in various development, implementation, data collation and evaluation of programmes to enhance the integration of care and strengthen collaborations with Nursing home partners in the NUHS Regional Health System. Prior to that, Rae was leading the National University Hospital Care Integration & Alliances (CIA) team, which manages the effective flow of patients from the restructured hospital to partnering healthcare institutes, from both inpatient & outpatient settings. This is achieved through close and continuous engagement with the stakeholders and patients, to address integration issues, and to eventually bring about system savings to the institute and better health outcomes for the patients. Being in the area of care integration since 2012, Rae started out with a strong focus on patient communication and project management, including time as a Care Coordinator (Right Siting Officer) offering financial counseling and advisory services to patients identified for transfer to primary care partners. In outpatient, Rae and her team of 8 primary care coordinators work alongside to perform appropriate siting of patients to the community, and pilot initiatives to reduce patients’ multiple appointments in NUH, with the intention to improve first visit wait time and to reduce SOC utilization. For inpatient, Rae works closely with colleagues from Community Hospitals such as St Luke’s Hospital, Jurong Community Hospital and Thye Hua Kuan Hospital (AMK) to facilitate transfers operationally and to lift traditional transfer and discharge planning limitations. Rae was also the secretariat of NUH's Longstayer Committee, where complicated cases with discharge planning issues are discussed and followup closely for effective escalation and learning.At the peak of the COVID-19 pandemic during the foreign workers’ dormitory outbreak, Rae volunteered to support NUHS Covid-19 management in the community. She led Mobile Swab Teams into the foreign workers’ dormitory to perform swab missions, and progress to perform roles in the NUHS Dormitory Operations HQ. Such roles include 24/7 logistics and analytic support to Dormitory Operations i.e. ambulance activation, ration, lab allocation and transport arrangement. In additional to that, Rae also contributed by providing onsite supervisory support for requesting nursing homes, and contact tracing with the NTFGH Epidemiology team.
