Social Services/Admissions Coordinator/Community Relations
Current- Conduct facility tours, promoting the facility and care provided
- Initial point of contact; ensure a positive resident experience throughout resident onboarding and transition process
- Conduct pre-admission assessments with nursing including financial assessments
- Successful onboarding including admissions documents and setup of new residents – PCC (Point Click Care)/Matrix and MDS assessments
- Establish relationships with all care partners such as the residents’ physician, nurse, caregivers and pharmacist
- Maintain the confidentiality and integrity of protected health information; follow HIPPA security policies and procedures
