• Conduct facility tours, promoting the facility and care provided• Initial point of contact; ensure a positive resident experience throughout resident onboarding and transition process• Conduct pre-admission assessments with nursing including financial assessments• Successful onboarding including admissions documents and setup of new residents – PCC (Point Click Care)/Matrix and MDS assessments• Establish relationships with all care partners such as the residents’ physician, nurse, caregivers and pharmacist• Maintain the confidentiality and integrity of protected health information; follow HIPPA security policies and procedures• Verify accuracy of new resident profile, including insurance information and work with prescribers to complete the intake process• Manage all communication with the resident and their designated POA • Coordinate care plan meetings with residents, families and interdisciplinary team to ensure proper plan of care• Complete forms for billing information and coordinate processing with finance for processing• Establish relationships with referral sources; promoting St. Louis Altenheim to hospitals and physician’s offices, resulting in developing and maintaining census.• Makes sales calls outside the facility to medical, insurance, legal and financial professionals, senior organizations, appropriate special interest groups, hospital discharge planners and other community contacts
