Duties include telephonically surveying a minimum of 36 Medicare, Commercial or Group Account members in seven domains of functioning; per week, evaluating member needs and requirements to achieve and/or maintain optimal wellness state; guiding members and their families towards, and facilitating interaction with, resources appropriate for the care and wellbeing of members. Position includes collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to manage member physical, environmental and psycho-social health issues. Also temporarily deployed to the Member Experience Team for 8 months. Results were a detailed review of approximately 500 telephonic contacts (at the rate of 6/day) made by Humana Associates with members. A detailed summary of the results, as graded by a objective scale, was provided to each Associate and supervisor. Out of the reviews, 0 were contested by the Associate or supervisor. Also, within that assignment, twice participated in special projects designed to review the results of Well-Being and Heart Healthy phone contacts. In each project, 100 calls were reviewed from Associates for 10 different variables. For both projects, a summary of the observations in addition to the specific metrics was submitted, unsolicited. . I specialized in integrating behavioral care into the workflow of two medical practices in the Baton Rouge area, leveraging technology and applying evidence-based protocols. I utilized a stepped care approach, with clinical data and analytics to guide patients to the right level of integrated medical and psychiatric care. Unfortunately for this area, the company was not able to sustain funding.
