This involves assessing the local situation, identifying key challenges, setting targets, and establishing a roadmap to achieve measurable outcomes.Partnership Coordination: Acting as a focal point for bringing together various stakeholders involved in addressing rough sleeping through fostering effective partnerships, encouraging collaboration, and coordinating joint efforts to maximise the impact of interventions.Data Monitoring and Analysis: Monitoring and analyzing data related to rough sleeping to understand trends, evaluate the effectiveness of interventions, and informing decision-making. Outreach and Engagement: Supporting outreach teams working directly with individuals experiencing rough sleeping. Going on outreach shifts, street counts and working closely with outreach workers to engage with rough sleepers, assess their needs, and provide appropriate support.Service Coordination: Coordinating services that support rough sleepers such as housing providers, healthcare professionals, social workers, public protection, day centres and other relevant agencies to ensure a coordinated approach to addressing the needs of rough sleepers, such as access to emergency accommodation, healthcare services, mental health support, and substance abuse programs.Advocacy and raising awareness about rough sleeping, homelessness issues, and the need for adequate resources and support. I also contribute to the development of local policies, guidelines, and procedures related to rough sleeping.
My role as a Rough Sleeping Assessment Hub Manager involves overseeing and managing the operations of the facility dedicated to assessing and supporting individuals experiencing rough sleeping or homelessness.I oversaw the day-to-day management of the assessment hub, ensuring the facility is clean, safe, and well-maintained. Coordination of staff and stakeholders to ensure the smooth operation of the hub.Overseeing a team of assessment and reconnection workers, volunteers and a management team including a deputy manager and case coordinator. Providing leadership, guidance, and support to the team, ensuring that they are well-trained and equipped to carry out their duties effectively.Coordinating the assessment process, ensuring it is conducted in a timely and accurate manner. Collaborating with assessors to develop individualized move on plans and connect individuals with relevant services for housing, healthcare, and social services.Developing and maintaining partnerships with local authorities, housing providers, outreach teams, and other organizations involved in addressing homelessness. Collaborating with stakeholders to enhance the effectiveness of the hub's operations and improve outcomes for individuals experiencing rough sleeping.Data Management and Reporting.The manager is responsible for maintaining accurate records of assessments, support provided, and outcomes achieved. Identifying areas for improvement within the assessment process and service delivery through feedback from service users and stakeholders to enhance the quality and efficiency of the hub's operations. Ensuring the hub operates in compliance with relevant policies, regulations, and funding requirements.
