I worked for the Partial Care department which uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served. Partial Care is a day treatment program for severely and persistently mentally ill patients who require structure, intensive treatment, and close medication monitoring to prevent hospitalization. At this position I provided case management and support to partial care clients, facilitated groups, documented according to regulatory standards, carried a caseload, functioned as part of the partial care team and maintained collateral contacts with family members and providers in the community on behalf of clients. Groups include: communications skills, assertive training, medication management, anxiety management, depression management, symptom management, health and wellness, social skills, wellness recovery action plan, and social skills.

As youth counselors at Bonnie Brae we provide supervision, counseling, crisis intervention, behavior management and monitoring of the basic needs for the boys who live in cottages on campus. We work with the interdisciplinary treatment team to develop individual treatment plans which guide our interactions with each resident. Responsibilities include daily supervision of youth, leading resident group meetings, coordinating recreation activities and family involvement, and providing a supportive living environment for youth in care. As a youth counselor we Also at times take the Youth off grounds on trips, these trips are based on behavior and how well they are doing with their treatment. Staff also assign goals to their primary youth, and document behavior and progression towards goals. We also keep in touch with families and/or cmo workers for each youth. We are responsible for the documentation of incidents when necessary and also implementing PCM restraint tactics

Running group sessions for adults with mental illness and substance abuse issues ages 18 and up, being able to assist and communicate with them, writing daily and weekly notes on their behavior and participation during group sessions, teaching different lessons to them on a daily basis. Make sure clients abide by the rules of the facility and encourage them to act accordingly and also motivate them to partake in the day by day lessons and activities. This job also requires me to create IRP’S (individual recovery plans) for each client in my case load when they are due, as well as write clinical notes on each client as necessary . Encourage clients to set and achieve goals, make referrals to see the psychiatrist, making sure clients understand their symptoms, diagnosis, and medication usage, as a counselor/case manager I must be able to communicate with clients boarding home staff if and when necessary, and also to observe and note the progress of each client in my caseload.
