Family Services Specialist
CurrentMonitor, coordinate, and administer programs and services within the areas of Adult/Adult Protective Services, Child Protective Services, Foster Care/Adoption. Assess client needs and other relevant factors such as education/skill levels, abilities, interests, and support systems. Provide information to clients for related service programs rules/regulations, and right to participate. Work collaboratively with local benefits program staff to determine available financial programs and assist in enrollment of clients in eligible benefits programs such as Medicaid, Childcare Assistance, VIEW/TANF, SNAP, or Emergency/Crisis programs to support their successful participation in family services prevention activities. Assess and investigate cases to determine appropriate services, develops appropriate services plans, and support a client's successful completion of treatment/service plan goals. Provide case management and intervention services to ensure the safety and well-being of youth and adults involved in the social service system, and the development of service plans for youth and families to connect them with appropriate services and/or treatment based on assessed strengths and needs. Coordinate with other local agencies and external stakeholders in the community and participate in multidisciplinary and interagency teams to build a support network for families at risk in the community to access necessary services. Serve as the Continuous Quality Improvement designee for Family Services to monitor the unit's performance in Critical Measures established at the state level. Monitor Safe Measures data reports for individual and unit level performance throughout the month, and coordinate with colleagues to share the unit's performance standards, identify data entry issues that affect performance reporting, and provide guidance for the appropriate entry of data according to policy to ensure accurate reporting. Certified by VDSS to facilitate Family Partnership Meetings.
