CRI provides Companion, In Home/Community, and Community Participation support to individuals with intellectual and developmental disabilities along with other physical disabilities. I oversee the service planning and implementation of outcomes for our consumers. I currently have close to 40 consumers with about 50 supports staff who work with them. As Program Specialist, I am responsible for monthly progress updates on all consumers as it relates to their outcomes in the Individual Support Plan. I work directly with other agencies such as Skills, ACEL, Crossroads, Empowering Lives Foundation, Innovative Support Solutions, and others on multi-disciplinary teams to ensure the best possible outcomes for my consumers. I also submit any updates to the Individual Support Plan to Service Coordinators as needed throughout the year. Another responsibility I have in this position is reporting and documenting all incidents to regional and state administrations through EIM (Enterprise Incident Management System) in HCSIS. Our goal is to help these individuals be as independent as possible and to have the confidence to feel empowered and be beneficial members of the community.

Named 2016 Nonprofit of the Year by the Chattanooga Chamber of Commerce, the Northside Neighborhood House (NNH) is a nonprofit organization located in Chattanooga, Tennessee.I worked in the Direct Assistance Program at NNH, which provides a hand up to families when they need it most. The DA program works in partnership with other agencies to provide services to clients including:- Financial assistance with emergency needs for utilities, food, and prescription medications- Case management with budgeting coaching- Referrals to other NNH and community programs as appropriate (work readiness, resume development, job -search help, interviewing skills, and an on-site GED/HiSET tutoring and acquisition)- Follow-up phone calls to check on clients’ progressServing the largest numbers of clients of any staff in Direct Assistance during my time as Case Manager, I personally managed over 415 unduplicated households with bills totaling over $107,900. $3,750 of those funds were through the congressional Emergency Food and Shelter Program (EFSP), funded by the Federal Emergency Management Agency (FEMA). Total food assistance accounted for 492 clients with more than 10,800 meals. I also coordinated Chattanooga's monthly CEAP (Coalition of Emergency Assistance Providers) meetings. We would meet with several local organizations that provide services to Chattanooga residents. Before working at Northside Neighborhood House, I had never served in a clerical setting before. This position has provided me with a new employment opportunity while continuing to serve in a nonprofit organization. My passion of serving people has expanded to fulfilling a new role as receptionist. I would schedule direct assistance appointments for our case managers, answer multi-line phones, transcribe and relay messages to executive and assistant directors, organize board meetings, and greet visitors to our administrative building.Some of my other tasks were scheduling follow-up appointments, answering telephone inquires about our thrift stores and other educational programs for children and adults, directing visitors to appropriate conference rooms for appointments, maintaining attendance reports for programs, and drafting internal and external correspondence, including letters, emails and memos to various community and board members.

Here at Volunteer I worked as both Discharge Planner and Psychiatric Technician for one of TDMHSAS's 8 Crisis Stabilization Units located across Tennessee. CSUs provide facility-based, voluntary services that offers 24-hour, seven day-a-week, intensive, short-term stabilization and behavioral health treatment for people 18 and older whose behavior health condition does not meet the criteria for involuntary commitment to a psychiatric hospital or other treatment resource.Volunteer's Crisis Stabilization Unit has recited CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation and is licensed through the Tennessee Department of Mental Health.In Discharge Planning, I worked with each client individually each day to assist them in the transition from in-patient care back into the community based on their strengths, needs, abilities, and preferences. These duties included; scheduling after care and follow up appointments with outpatient clinics, referring clients to various Alcohol and Drug treatment options for those with co-occurring disorders, referrals to Primary Care Physicians, Peer Support education, and assisting clients without insurance in applying for Behavioral Health Safety Net if eligible.As a Psych Tech, my primary role was providing hands-on care to people with varying degrees of mental illnesses and/or developmental disabilities. I also helped conduct educational, recreational, and therapeutic activities on the unit, observe patients to detect behavior patterns then chart and report to medical staff, contact patient’s family or friends by telephone to arrange conferences or visitations if desired, and complete initial admittance forms for new patients.

Formerly Fortwood Center, I worked at this location for 2 years and was promoted to supervisor after only 1 year of service in the adult case management program in Hamilton County. I supervised a team of 15 community case managers for adult clients with severe, persistent mental illness. After my promotion, this team continually saw improved objective, measurable progress with our clients every month following (an average of 9 GAF points increase per client served), as well as increase to access of services for clients without insurance from 210 clients served per month, to over 300.I regularly interacted with several local psychiatric hospitals for clients during their inpatient treatment at Moccasin Bend Mental Health Institute, and multiple Parkridge Valley campuses. I collaborated personally with Chattanooga's city mayor, Andy Berke, as well as several other community partners to provide the most outstanding, quality care to the mental health population in Chattanooga.Some of my direct duties as supervisor were; maintaining an active role as a community case manager half time and direct supervision of my staff the other half, assigning appropriate case managers to clients, quality assurance and quality improvement of client care throughout the Adult and Children Youth and Family clinics in Hamilton County, referring clients to services both in-house and in the community, maintaining and budgeting client assistant grants through the Department of Mental Health and Substance Abuse Services, as well as public speaking and representation of HRMC throughout the East Tennessee area both in person and on camera for local news interviews.The Helen Ross McNabb Center was the first community mental health center awarded CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation in the state of Tennessee 16 years ago. The most recent 3 year accreditation (the most stringent) is the seventh consecutive awarded to the Center.

At one of my most enjoyable jobs, I provided supportive services to clients with severe, persistent mental illness, including: coordination of care, advocacy, maximizing natural supports, and treatment planning of goals based on their strengths, needs, abilities, and preferences. I met with clients throughout the community and in their homes to help those living with severe mental illness overcome barriers and cultural stigma. While working for STS I transported clients to and from outpatient appointments (both physical and behavioral), pharmacies, and discharges home from inpatient care facilities. I functioned under the DOT safety and operating guidelines for patient travel; clients both independently walking and those that used mobility devices (cane, walker, wheelchair, etc.).

Employed through New City Fellowship, I served as the 1st and 2nd grade counselor/teacher for the ELECT (East Lake Every Child Taught) summer program sponsored through the Hamilton County YMCA locations. I also directed behavior and discipline for students throughout the program (ages 5 - 12). Students were tutored in math and reading in compliance with current Common Core standards as well as Hamilton County's Board of Education.

As training coordinator I trained new employees throughout the Hamilton and Bradley County district locations, advised district and division managers during the hiring and termination process, as well as maintained an active role in start-to-finish customer service at several restaurant locations. I was approached by several managers throughout my career recommending me to pursue a management role.
