Current•Create an environment of trust to provide participants a single point of contact to overcome participants’ perceived barriers throughout participation in the FACT (Fathers and Children Together) Program.•Coordinate service plans across multiple systems.•Obtain eligibility documentation and secure voluntary informed consent and confidentiality agreements.•Customize and schedule written plans of services with the participant taking into consideration strengths, weaknesses, schedule and input.•Manage participants detailed files and database information with case notes, supporting documentation, monthly assessments and all end of services documentation.•Accompany or assist participants to services as appropriate and necessary. •Provide ongoing encouragement , support and assistance to participant through service plan •Participate in regular Mentor Navigator team staffing to facilitate effective cross system navigation to identify and resolve services gaps.
