• Counsel with parents at time of enrollment to give a broad view of the Head Start Program and serves as an advocate as well as providing assistance in meeting the needs of the family• Conduct home visits to identify and assist families with needs as an ongoing, continuous basis• Complete Individualized Partnership Agreements with each family to help family achieve short- term and long-term goals• Assist, arrange, and provide transportation for parents/guardians on any needed medical, dental, or Community Resource appointment when the need is identified• Serves as a liaison by connecting families and providing support and referrals to other community agencies/resources• Assist in on-site health services to children by contracted service agencies that may include: assembling children, accurately processing paperwork, performing routine procedures as directed by the Service Contractor
