As the Admissions Coordinator, my primary responsibility is to plan, coordinate, and manage the admissions process for our facility, ensuring a seamless and positive experience for residents, families, and staff. I oversee the daily operations of the Admissions Department, working collaboratively with other facility leaders, and maintaining a comprehensive understanding of our services. My role also includes facilitating daily admissions, coordinating with nursing staff, maintaining confidentiality, and assisting in outreach events. I strive to create a warm, positive environment and adhere to all compliance standards.- Efficient Admissions Management: I manage the admissions process with efficiency and professionalism, ensuring that residents are admitted in a timely manner and that their experience is positive and well-coordinated.- Strategic Collaboration & Outreach: I work closely with physicians, social workers, and discharge planners to maximize referrals and facilitate admissions. My role also includes planning and executing promotional events to attract key stakeholders to our facility.

- Work on-site at partner hospitals or skilled facilities, to identify and enroll members into the Care Transition program- Contact healthcare provider(s)/care manager(s) to establish documentation of transitional care plan- Follow member through the continuum of care (hospitalization/SNF) with home and facility visits, and perform follow up telephone calls

I was assigned to facilitate discharge-related transition support services, generally based upon the Coleman Care Transition Intervention.
