In my current job role I work for Dudley CVS as a Senior Social Prescriber for the Integrated Plus service. In this role I provide holistic personalised support for patients with non-clinical support needs. Referrals come from GPs surgeries and other healthcare practitioners who are part of local MDT meetings. As a senior social prescriber in my locality, I represent a wider team of social prescribers at weekly Community Partnership Team meetings. This involves working collaboratively with representatives from GP surgeries, mental health services and social services to ensure the best outcome for patients. As a senior worker I also help set-up and run Connect and Share events that Integrated Plus hosts bimonthly. These events provide information to social prescribers and professional in the locality. It also offers the chance to access peer support. I also host biweekly peer support sessions with social prescribers from the Primary Care Network to support with any issues they are experiencing and carry out case reviews. As well as the above-mentioned duties, whilst working for Integrated Plus I have taken on extra responsibilities managing the services social media account and in 2022 started a podcast called Talking Social Prescribing. I am also a member of the National Academy for Social Prescribing's Innovation Network and one of their Social Prescribing Champions.

As a Social Prescribing Link Worker I work with patients presenting with non-clinical factors affecting their health and wellbeing referred by General Practitioners and healthcare professionals. This job role entails providing person-centred support to patients experiencing social and psychological difficulties. This role is part of the NHS's model for Universal Personalised Care and ensures that the GP surgeries are able to provide support for non-clinical issues affecting their patients wellbeing and provide holistic personalised care. Initial challenges of this job role was the design and implementation of the service across the Primary Care Network. This included embedding myself into different GP practices within the Primary Care Network and attending meetings to promote the service to all healthcare professionals. Skills utilised during each appointment with patients include: active listening, counselling techniques, health coaching, motivational interviewing and using a psychologically informed approach when dealing with emotive conversations. Whilst working with each patient it is vital that a positive working relationship is established that enables disclosure. A key part of this job role is also to liaise with doctors, medical staff who make referrals and also build relationships with CVS organisations to whom I refer patients. Data is collected regarding referrals from GPs, referrals made to external services and wellbeing of patients during their time engaging with the service. This role also requires excellent caseload and time management.

As a Lead Worker I managed a caseload of clients with multiple and complex needs. Referrals were accepted from multiple agencies for people experiencing three of the four: homelessness, addiction, offending and mental health. In this multifaceted role I worked both independently and with peer mentors. Using a multi-agency approach a Lead Worker would coordinator wrap around support for the client, liaise with services, and provide appropriate personalised support. Outcomes relating to each client and their ongoing support needs were monitored using monthly data submissions and analysis. Other duties of the role were: carrying out initial assessments, completing risk assessments, developing dynamic support plans, and liaising with other services on behalf of the client to ensure their needs were being met. As a Lead Worker I also managed and supervised a group of peer mentors who used their lived experience to engage with and support clients.

Working for Sense as a Multi-Sensory Impairment Practice Adviser as part of the Sense Practice Development Team I used my specialist knowledge of sensory impairment and organisational psychology within services, to staff and management. A key duty was to visit services and carry out a variety of assessments. In this role I was required to work independently managing my own calendar and workload as the role was home based. Assessments related to the care needs of people Sense provide services for and helping to maintain the quality of staff practice. Other duties of the role included: training staff, practice observations, writing support guidelines and carrying out functional assessments. The role required me to work independently and be able to build positive relationships quickly due to the nature of going to several different services in a short period of time. A high level of interpersonal sensitivity and the ability to communicate appropriately was utilised. Another aspect of the job role was carrying out staff training sessions, both formally and informally. This included sessions on a one-to-one basis or presenting training to small and large groups. Training sessions often needed to be adapted to suit the needs of the learner due to their communication or learning difficulties.

Providing a high level of person centred support for a variety individuals with a range of sensory impairments and intellectual disabilities. The varied nature of this role developed a variety of skills both knowledge based and interpersonal. As well as supporting a variety of people with complex needs I worked in different service and was able to integrate into staff teams within a short space of time. This requires the ability to retain detailed person-centred information from care plans in order to work effectively and provide the best levels of care possible. Throughout my employment with Sense I developed skills training new staff, auditing/ordering medication, taking an active role in multidisciplinary meetings and developing new ways of working with clients based on knowledge regarding their idiosyncratic needs. I also managed my time effectively whilst working almost full time hours and studying for my undergraduate and postgraduate degree.

My role at Clive Mark was an administrative and sales role that included processing orders, quality control and dealing with financial audits. During this time I helped to develop contacts to broaden the customer base and ensure the quality of all deliveries in and out of the warehouse.
