In my current job role I work for Dudley CVS Integrated Plus service as a Senior Social Prescriber providing holistic personalised support for patients with non-clinical support needs. The Integrated Plus service receives referrals from GPs & healthcare practitioners for people experiencing practical, social and emotional difficulties that affect their wellbeing.Other duties that I have undertaken in the role of social prescriber include co-writing successful bids for contracts, both for project funding & PCN contracts to provide a social prescribing service. I have also designed and delivered training for staff new to the role. In my current role I have undertaken extra duties that include managing the services social media platform and hosting a podcast to promote the service.

In this role I provide holistic personalised support for patients with non-clinical support needs. Referrals come from GPs surgeries and other healthcare practitioners who are part of local MDT meetings. As a senior social prescriber in my locality, I represent a wider team of social prescribers at weekly Community Partnership Team meetings. This involves working collaboratively with representatives from GP surgeries, mental health services and social services to ensure the best outcome for patients. As a senior worker I also help set-up and run Connect and Share events that Integrated Plus hosts bimonthly. These events provide information to social prescribers and professional in the locality. It also offers the chance to access peer support. I also host biweekly peer support sessions with social prescribers from the Primary Care Network to support with any issues they are experiencing and carry out case reviews. As well as the above-mentioned duties, whilst working for Integrated Plus I have taken on extra responsibilities managing the services social media account and in 2022 started a podcast called Talking Social Prescribing. I am also a member of the National Academy for Social Prescribing's Innovation Network and one of their Social Prescribing Champions. -

This job role entails providing person-centred support to patients experiencing social and psychological difficulties. This role is part of the NHS's model for Universal Personalised Care and ensures that the GP surgeries are able to provide support for non-clinical issues affecting their patients wellbeing and provide holistic personalised care. Initial challenges of this job role was the design and implementation of the service across the Primary Care Network. This included embedding myself into different GP practices within the Primary Care Network and attending meetings to promote the service to all healthcare professionals. Skills utilised during each appointment with patients include: active listening, counselling techniques, health coaching, motivational interviewing and using a psychologically informed approach when dealing with emotive conversations. Whilst working with each patient it is vital that a positive working relationship is established that enables disclosure. A key part of this job role is also to liaise with doctors, medical staff who make referrals and also build relationships with CVS organisations to whom I refer patients. Data is collected regarding referrals from GPs, referrals made to external services and wellbeing of patients during their time engaging with the service. This role also requires excellent caseload and time management. -

Referrals were accepted from multiple agencies for people experiencing three of the four: homelessness, addiction, offending and mental health. In this multifaceted role I worked both independently and with peer mentors. Using a multi-agency approach a Lead Worker would coordinator wrap around support for the client, liaise with services, and provide appropriate personalised support. Outcomes relating to each client and their ongoing support needs were monitored using monthly data submissions and analysis. Other duties of the role were: carrying out initial assessments, completing risk assessments, developing dynamic support plans, and liaising with other services on behalf of the client to ensure their needs were being met. As a Lead Worker I also managed and supervised a group of peer mentors who used their lived experience to engage with and support clients.
