Over the last 15 years, I've built and led teams, organizations and coalitions to drive radical health improvement for patients and communities. After demonstrating dramatic improvements in care and reductions in emergency department visits and hospital admissions, I subsequently managed the clinical expansion of the program, helping to establish and lead six additional primary care teams, an 80,000 sq.ft one-stop shop for health and human services, and a robust community engagement strategy, including a local and national medical-legal partnership strategy for homeless Veterans. The care model we established was formally recognized as “the highest quality and most compassionate care I have ever witnessed” by Dr. Thomas Bodenheimer, Emeritus Professor and founding Director of Center for Excellence in Primary Care, UCSF.
