This includes performing comprehensive client assessments to determine client needs and problems in relation to their physical and psychosocial wellbeing, developing care plans with clients and/or their primary caregiver to meet those needs, and reviewing and monitoring the outcomes of interventions implemented to address client's medical health problems and mental health, strengthen social support, and increase access to community, housing/environmental, vocational, and educational resources.
