For more than 12 years I have worked in Community Services, mostly for a very large NGO where I worked at various times in case-management, team-leadership and senior-leadership roles.Consequently, I have accrued vast knowledge and practical experience across a wide range of cognitive, intellectual, psychosocial and some physical disabilities. Thanks to my leadership role during the transition to NDIS, I also have an intimate knowledge and understanding of that system and am therefore able to offer highly insightful and skilled planning, co-ordination, and delivery of quality, evidence-based support to people with whom I have the privilege of working. My proven ability to work professionally with a wide range of government and non-government agencies, as well as families and other valued stakeholders, ensures that the strong partnerships I’m able to build further enhance the quality of all-round support provided and truly maximise the potential for outstanding participant outcomes.However, above all else, it’s my natural ability to keep things ‘real’, thanks to far-reaching life experiences prior to entering community services, that truly lies at the heart of my support. Over the years, I have enjoyed a range of occupations and a multitude of tremendous experiences for which I’m very grateful and I consider myself extremely fortunate to have lived such a very rich and varied life of my choosing. It is therefore extremely important to me that the work I do with others maximises their opportunities to pursue their own adventures, or ambitions, in the same way that I was always free to pursue mine. • Oversee all onsite client support including coordination of (2) separate programs in a 22-bed residential facility with a total of approximately 90 staff.• Develop a high level of understanding and proficiency in key areas including NDIS - including SIL and a recent expansion of our service towards an outreach SIL model.• Provide case reports and other information as required and maintain records and collate statistical data in accordance with requirements of the Service.• Support the delivery of safe client care and continuous quality improvement activities in accordance with the requirements of the Service Standards.• Use a variety of interventions, with individuals, families and groups and develop a strong knowledge of community services available in the area to support clients. • Act as a facilitator between clients in need and community services and assess and refer to health, welfare, recreation, housing, employment, and other community services as needed. • Implement best practice policies using practice experience, research, analytic frameworks, and negotiation skills to respond to social need through a fair, equitable and effective allocation of social resources.

In this position I mentored and supported a team of case managers working with people living with an intellectual, cognitive and or psychiatric disability in a long term accommodation program by utilising a strength-based, empowerment model. • Support clients living with an intellectual, cognitive and and/or psychiatric disability to become more independent and empowered through a recovery-based service model. • Provide case management and community development interventions to individuals living with an intellectual, cognitive and or psychiatric disability in keeping with the guidelines contained in the service agreement and plan.• Conduct regular planning reviews with clients and ensure that plans are monitored and progressed and develop, in conjunction with the Team Leader, continuous quality improvement systems and quality services and supports to individuals living with a disability and their families. • Maintain an environment that empowers clients and promotes positive behaviours, positive learning, skills development, and independence with effective communication with relevant stakeholders.
